If you are interested in joining ASAPP, please fill out and fax or mail the necessary attached membership forms. There are a total of 6 forms including a credit card form to make the initial payment using your credit card (checks are also excepted if you prefer). The total initial cost to join ASAPP is $240.00 for a primary member. This amount includes the first and last months payment ($70 per month) and a $100.00 on time admin fee.
If you have any employees, please state how many employees to be included on your policy including their full names on the application. Each employee is an additional $50.00 per month. The initial start-up cost for each employee is $175.00. This amount includes the first and last month payment ($50.00 per month) and a one time $100.00 admin fee.
Once we have received the completed membership forms and payment, the policy will be fully processed. As soon as the forms are processed, ASAPP will mail out the insurance binder and a copy of the receipt.
